If you receive notification from a Board of Cosmetology regarding your application, that notification is the date the Board receives the application. Cosmetology license applicants must pay the application fees on-time. If the fees are not paid the licenses are not issued. It was not previously disclosed to applicants that the application fees were non-refundable and are due at the time the application is submitted. Applicants can pay on-time by credit card or cash in the Cosmetologist's office. Please see attached "Pay Here to Apply" form for details. Please read the instructions before completing this form. The completed form must be mailed to the address provided below. Your application will take 3-4 weeks to be processed - once processed, you will have two weeks to arrange to receive your license. If applicable, a Cosmetologist Certification Statement is also required. Your application will be reviewed at my board location. By mailing the completed application, applicants: (1) certifies that they will be familiar with and comply with all of the rules and regulations of the Board of Cosmetology. (2) certifies that they are not an attorney licensed in this state. If in doubt, please call your city or county Board of Cosmetology, contact the Board of Cosmetology, or return this form to the Cosmetologist. Failure to return a form to the Cosmetologist will delay the process. Applicants must submit information and pay the application fees on time. Failure to do so may result in the license application not being processed and licenses not issued. The information on the application is the same as that contained on the application with the exception of the Cosmetologist certification statement. You will receive a Certificate of Registration in three separate stages: $60 - first stage $120 - second stage $180 - final stage *Once the application is received the Cosmetologist will contact you with the next steps after you pay your application fees. The Cosmetologist has two weeks to complete the Cosmetologist Certification Statement from the date you post payment for all three stages. If applicable, if you receive notification from a Cosmetologist regarding your application, that notification is the date the Board receives the application.
